Instructions: For each question, enter a number 0-5 on the score sheet that best describes you.

  • 0 = Never
  • 1 = Seldom
  • 2 = Sometimes
  • 3 = Usually
  • 4 = Mostly
  • 5 = Always

Some helpful tips:

  • Go with your first impression. The longer you think about the question and your answer, the less accurate it will probably be.
  • Do not put down what you think you “should” be doing. Be truthful and accurate for the best results.
  • Be consistent with your ratings

Once you are complete, add up the totals for each column, and then go through the descriptions of the different gifts below.
